All forms must be completed prior to your appointment. While client photos are never used without permission\, completion of all intake forms is required to proceed with services. \nAppointment Timing: To allow adequate time for your service\, we kindly ask that all forms be completed in advance and that you arrive a few minutes early if needed. If forms are incomplete at the time of your appointment\, there may not be enough time to safely perform the service. \nCommunication: If we are unable to confirm your appointment or receive completed intake forms ahead of time\, your appointment may need to be removed from the schedule.
