Your Care Team

Our Michelle’s Place Team will quickly become your family.

At Michelle’s Place Cancer Resource Center, you will be greeted by a care team that will guide you and provide a safe place of comfort through your journey.

The diagnosis of cancer comes with so many unanswered questions and concerns. It can be overwhelming, and the next steps are not always clear. There is one thing that is certain- a diagnosis will require support and someone to listen and understand. Someone to help you talk through your major concerns and link you to resources and support systems to ensure you do not “face cancer alone”.

Your Care Team at Michelle’s Place consists of Patient Navigators, Community Health Workers, Program Facilitators, and Behavioral Health professionals.

Care Team – Patient Navigators

A patient navigator will listen to your story, and together you will access your needs, and develop a plan to meet those needs. They will answer any questions that we can about the next steps and be there for them as support. They will link you to resources that most fit your needs, and help you plan for upcoming surgeries, treatments, and much more. There is an abundance of resources that are community and nationally based, but it can be daunting to search for them. Your Patient Navigator will do this for you. Whether you have educational, financial, or support needs, we are here for your journey.


Community Health Worker

Our Community Health worker works hand in hand with your navigator to provide additional resources based on certain factors as defined in the Social Determinants of Health. These factors include the access and quality of education, health care, your environment, social support, and economic stability. These resources are tailored to your specific needs and your journey. A Community Health Worker is also trained to perform home visits if you are immobile and accompany you on doctor visits as needed.
