Intake Forms: After your appointment is scheduled, you will receive a text confirmation with a link to complete required electronic intake forms, including a clinical/medical consent form and a photo consent form (paper forms are not available). All forms must be completed prior to your appointment. While client photos are never used without permission, completion of all intake forms is required to proceed with services.
Appointment Timing: To allow adequate time for your service, we kindly ask that all forms be completed in advance and that you arrive a few minutes early if needed. If forms are incomplete at the time of your appointment, there may not be enough time to safely perform the service.
Communication: If we are unable to confirm your appointment or receive completed intake forms ahead of time, your appointment may need to be removed from the schedule.
