At Michelle’s Place Cancer Resource Center, you will be greeted by a care team that will guide you and provide a safe place of comfort through your journey.

The diagnosis of cancer comes with so many unanswered questions and concerns. It can be overwhelming, and the next steps are not always clear. There is one thing that is certain- a diagnosis will require support and someone to listen and understand. Someone to help you talk through your major concerns and link you to resources and support systems to ensure you do not “face cancer alone”.

Care Team – Patient Navigators

A patient navigator will listen attentively to your story, working together with you to assess your unique situation and formulate a plan to address them. They are here to provide answers to your questions about the next steps and to offer unwavering support along the way. They will connect you with resources that best suit your needs and assist you in preparing for upcoming surgeries, treatments, and more. Navigating the abundance of community and national resources can feel overwhelming, but your Patient Navigator will handle this for you. Whether you require educational guidance, financial assistance, or emotional support, we are committed to supporting you throughout your journey with compassion and warmth.

Community Health Worker

Our Community Health workers work closely with your navigator to improve your care by addressing key factors outlined in the Social Determinants of Health. These include access to education, quality healthcare, environmental conditions, social support networks, and economic stability. We tailor resources to fit your specific needs and circumstances. Our Community Health Workers are also prepared to conduct home visits for those who cannot travel and accompany you to medical appointments when required. This holistic support is designed to empower and assist you through all aspects of your health and well-being.
